About the role

selectability is seeking a compassionate and experienced nurse to join the team in Townsville. This role focuses on supporting individuals as they transition from hospital to home or residential care. You will work closely with individuals, families, and healthcare professionals to develop and implement therapeutic care plans, coordinate services, and ensure smooth discharge planning.

Key responsibilities include but are not limited to:

  • deliver person-centred care plans tailored to each individual’s recovery goals
  • provide expert wound assessment, dressing, and management, ensuring optimal healing outcomes
  • collaborate with allied health professionals, families, and hospital discharge teams
  • monitor clinical progress and maintain accurate documentation in line with best practice

About you

To be considered for this rewarding role you will have:

  • highly developed person-centred communication skills
  • excellent communication skills, both verbal and written
  • a demonstrated high level of attention to detail with a consistent approach
  • a demonstrated ability to prioritise conflicting priorities as required
  • ability to work under minimal supervision with the capacity to exercise initiative in the application of work processes and procedures and establish objectives and outcomes
  • a demonstrated ability to show empathy and compassion
  • a proven ability to think critically and approach challenges with innovation
  • positive, can-do attitude

Role Requirements

  • diploma in nursing
  • AHPRA registration to practice as a nurse in Australia
  • current nationally recognised first aid and CPR certificates (or ability to obtain)
  • a Positive Notice Working with Children (blue card) (or the ability to obtain)
  • NDIS Worker Screening Clearance (or the ability to obtain)
  • NDIS Worker Orientation Module "Quality, Safety & You" completion certificate (or the ability to obtain)
  • a current minimum P1 "C" class Queensland drivers licence
  • a roadworthy vehicle with comprehensive insurance 
  • current flu vaccination (desirable)
